The Free 925(c) Document Checklist

Every document DOJ’s official application form requires, in one page — updated September 16 after we read all 17 pages of the form — with Kentucky-specific notes on where to get each one and how long retrieval really takes. The first public application window (November 4) is capped at 5,000 nationwide; the slow items take weeks, so start now.

What's on it

• Certified court records for every conviction — judgment, charging document, and proof your sentence (including supervision) was completed
• Local arrest records from every place you've lived in the last 25 years (or since age 18) — even localities where you were never arrested. The form's biggest surprise, and the slowest item.
• Fingerprints — two FD-258 cards or an equivalent electronic scan, and where to get them in Kentucky
• Character references: the three-year rule, who is disqualified from serving, and every certification on DOJ's reference sheet
• The two signed authorizations — general release and mental-health release
• Local chief law-enforcement notification — DOJ's template letter and how to document it
